Isolated sphenoid sinusitis complicated by meningitis and multiple cerebral infarctions in a renal transplant recipient.

چکیده
Infection was amongst the most feared complications of renal transplantation in the early era of this technique. Serious infection developed in up to 70% of renal transplant recipients, and a subsequent fatal outcome occurred in as many as between 11 and 40% [1]. Improved surgical techniques, more targeted immunosuppression and prophylactic antimicrobial agent use have led to a dramatic reduction in these rates. Nevertheless, infection remains a leading cause of morbidity and mortality at all stages of the posttransplant period [1]. Infections in this population are often unconventional and opportunistic, and therefore active surveillance for infection remains a cornerstone of management. We report a case in which isolated sphenoid sinusitis in a renal transplant recipient led to multiple complications and an ultimately fatal outcome.

Sphenoid Sinusitis Complicated by Pneumococcal Meningitis and an Infectious Aneurysm in the Intracavernous Carotid Artery.
A 61-year-old Japanese woman presented with a headache and appetite loss lasting for nine days and was admitted to our hospital, where she was diagnosed with pneumococcal meningitis associated with acute sphenoid sinusitis. While the administration of meropenem and dexamethasone ameliorated the meningitis, right third and sixth nerve palsy suddenly developed 10 days after admission. CT angiogra...

We describe a case of invasive fungal sinusitis caused by Scytalidium dimidiatum in a lung transplant recipient. Treatment was complicated by renal failure with amphotericin B therapies. Following 6 months of voriconazole treatment, the patient remained radiographically and clinically stable for a short time before dying of respiratory failure precipitated by graft rejection.

Pneumocephalus is a rare but important complication of acute isolated sphenoid sinusitis (ISS). If not adequately treated, it may cause serious neurological and life-threatening complications. We report the presence of free air in the cavernous sinus arising from intracranial extension of acute ISS. A 41-year-old healthy man presented to our department with a 7-day history of headache. Neurolog...
متن کاملAcute isolated sphenoid sinusitis.
INTRODUCTION Acute isolated sphenoid sinusitis is seen in fewer than 3% of all cases of sinusitis. It is frequently misdiagnosed because of its vague symptoms and the paucity of clinical findings. We report 2 cases of isolated acute isolated sphenoid sinusitis with unusual presentations. CLINICAL PICTURE Both patients presented with acute headache, eye pain and fever, and were provisionally d...
متن کاملFulminant bacterial meningitis complicating sphenoid sinusitis.
Intracranial complications of sinusitis, including meningitis, are uncommon in childhood. Among pediatric patients admitted for treatment of sinusitis, 3.2% were found to have an intracranial complication. Conversely, among pediatric patients with intracranial infections, only 2.4% had infections secondary to sinusitis. Infection of the sphenoid sinuses, however, merits particular concern. Thes...
